Comparative characteristics of the main approaches used in total knee arthroplasty

Abstract

The choice of surgical approach in total knee arthroplasty remains a subject of ongoing debate. This review provides a comparative analysis of five major approaches — medial parapatellar, subvastus, midvastus, lateral parapatellar, and quad-sparing — with respect to technical characteristics, perioperative outcomes, and functional results. Quadriceps-sparing approaches demonstrate advantages in the early postoperative period and are compatible with Enhanced Recovery After Surgery (ERAS) protocols; however, long-term functional outcomes are comparable across all approaches. The selection of the optimal surgical approach should be guided by individual patient characteristics, the degree of deformity, and the surgical experience of the operating team.
