Research methods: 1) clinical objective research (questionnaire, case history analysis, height and weight measurement with BMI calculation, blood pressure measurement, evaluation of skin condition and condition of nerves of the lower limbs); 2) laboratory tests (glucose, creatinine levels, LDLs, HDLs, triglycerides, total and active B12 level, glycolyzed hemoglobin in total blood, total urine test (albuminuria)) and instrumental tests (electromyography).</p> <p><bold>Results. </bold>One year later, the subjects recorded a decrease in HbA1c by 1% in the group with type 1 and type 2 diabetes with sensorimotor DN (from 9.0% to 8.0%) (p &lt; 0.001), and by 1.5% in the group with type 1 and type 2 diabetes, having the sensorimotor and neuropathic form of diabetic foot syndrome with trophic ulcer (from 9.0% to 7.5%) (p &lt; 0.001). There was also a decrease in BMI by 0.5–1.0 kg/m<sup>2</sup> (p &lt; 0.001), a decrease in LDL by 0.5–0.6 mmol/l, and an increase in HDL by 0.1 mmol/l, especially in the group with type 1 and type 2 diabetes, with sensorimotor DN and neuropathic form of diabetic foot syndrome with trophic ulcer, where 73% received statins. A significant decrease in neurological symptoms was observed in patients with type 1 and type 2 diabetes with sensorimotor DN (by 51% on the TSS scale (p=0.00005), by 46% on the NIS-LL scale (p=0.00003) and in patients with type 1 and type 2 diabetes with sensorimotor DN and the neuropathic form of diabetic foot syndrome with trophic ulcer (TSS increased by 68% (p=0.00007), on the NIS-LL scale by 45% (p=0.00004)).</p> <p><bold>Conclusion.</bold> Glycemic control, lipid-lowering and antihypertensive drugs, and timely treatment led to improved metabolic, neurological, and anthropometric parameters and faster healing of leg ulcers in patients with type 1 and type 2 diabetes.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель: </bold>изучить взаимосвязи между интенсификацией диспансерного наблюдения и динамикой лабораторно-инструментальных показателей пациентов с сахарным диабетом, имеющих сенсомоторную дистальную нейропатию (ДН) и нейропатическую форму синдрома диабетической стопы (СДС).</p> <p><bold>Материал и методы.</bold> В группу исследования вошли 150 человек в возрасте от 18 до 70 лет – мужчины и женщины с СД 1 и 2 типов, имеющие сенсомоторную ДН и нейропатическую форму СДС. Были проведены клиническое объективное исследование (опрос, сбор анамнеза, измерение роста, веса с вычислением ИМТ, измерение АД, оценка состояния кожных покровов и нервов нижних конечностей); лабораторные исследования (глюкоза, креатинин, липопротеиды низкой и высокой плотности, триглицериды, общий и активный витамин В12; гликированный гемоглобин в цельной крови, общий анализ мочи (альбуминурия); инструментальное обследование (электронейромиография).</p> <p><bold>Результаты. </bold>Через год у обследуемых при интенсификации диспансерного наблюдения и приеме гликлазида МВ фиксировали снижение HbA1c на 1% в группе с СД 1 и 2 типов, имеющих сенсомоторную ДН (с 9,0% до 8,0%) (р &lt; 0,001), и на 1,5% в группе с СД 1 и 2 типов, имеющих сенсомоторную ДН и нейропатическую форму СДС с трофической язвой (с 9,0% до 7,5%) (р &lt; 0,001).Также отмечали уменьшение ИМТ на 0,5–1,0 кг/м² (p &lt; 0,001), снижение ЛНП на 0,5–0,6 ммоль/л (p &lt; 0,001) и рост ЛВП на 0,1 ммоль/л (p &lt; 0,001), особенно в группе с СД 1 и 2 типов, имеющих сенсомоторную ДН и нейропатическую форму СДС с трофической язвой, где 73% получали статины. Значимое снижение неврологических симптомов отмечалось у пациентов с СД 1 и 2 типов, имеющих сенсомоторную ДН, – по шкале TSS на 51% (p &lt; 0,001), по шкале NIS-LL на 46% (p &lt; 0,00) и у пациентов с СД 1 и 2 типа, имеющих сенсомоторную ДН и нейропатическую форму СДС с трофической язвой, – по шкале TSS на 68% (p &lt; 0,001), по шкале NIS-LL – на 45% (p &lt; 0,001).</p> <p><bold>Заключение.</bold> Контроль гликемии, гиполипидемические и антигипертензивные препараты, своевременное обращение привели к улучшению метаболических, неврологических, антропометрических показателей и более быстрому заживлению язв на ногах у пациентов с СД 1 и 2 типов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>diabetes mellitus</kwd><kwd>distal neuropathy</kwd><kwd>diabetic foot syndrome</kwd><kwd>carbohydrate metabolism</kwd><kwd>fibular nerve</kwd><kwd>tibial nerve</kwd><kwd>gastrocnemius nerve</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>сахарный диабет</kwd><kwd>дистальная нейропатия</kwd><kwd>синдром диабетической стопы</kwd><kwd>углеводный обмен</kwd><kwd>малоберцовый нерв</kwd><kwd>большеберцовый нерв</kwd><kwd>икроножный нерв</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Sun H, Saeedi P, Karuranga S, et al.
