Identificationof diastolic dysfunction ofthe right ventricle at stage Iat hypertensive disease(clinical case)
Abstract
A case of essential hypertension (EH) stage I in a man at the age of 30 without concomitant
clinically significant pathology is presented. During the echocardiographic examination
(echocardiography), the standard method has been found diastolic dysfunction (DD) of the
right ventricle (RV). At the preclinical stage of structural and functional remodeling of the
heart at stage I of EH, before the right ventricular hypertrophy (RVH), diastolic dysfunction
(DDF) of the left ventricle (LV) and left ventricular hypertrophy (LVH) in the patient who had
DDF RV type of abnormal relaxation.

Dynamics of cardio-renalrelationship in patientswith CHF FC I-IIIundergone surgicalmyocardial revascularization
Abstract
The paper presents the results of a comparative study of structural and functional
parameters of cardiac performance and functional state of kidneys in patients with CHF FC
I-III, after myocardial infarction. In patients with heart failure who underwent surgical
revascularization, compared with patients who received only medical treatment, revealed the
better indicators of systolic and diastolic cardiac function, and also recorded an improvement
in tubular, glomerular renal function and reducing microalbuminuria.

Kinetics of musculartype arteries in patients with diabetestypes 1 and 2
Abstract
This is a study of biomechanics of muscular type arteries through a computer sphygmography
in patients with diabetes types 1 and 2. The study included 60 diabetic patients and 40 healthy
individuals. All respondents were divided into 3 groups: group 1 (control) were apparently healthy
people 2 - patients with type 1 diabetes, 3 - patients with type 2 diabetes. The study assessed the
speed characteristics of the arterial wall displacement and the duration of the phases of the
vascular bed. Analysis of the kinetics parameters of the great arteries showed the common
qualitative and quantitative changes in propulsive activity of the vascular wall in patients of
both types of diabetes. However, in type 2 diabetes these changes are more marked.

Hypertensivecrisis: effectiveness medicinetherapy assessmentat pre-hospital medical care
Abstract
The analysis of statistical data by MMU Center Ambulance Samara-city in 2008, 2009, 2010,
revealed that the number of calls about the HC is steadily increasing, hospitalization rates
have decreased in 2010. A retrospective analysis of the effectiveness of treatment of patients
with uncomplicated hypertensive crisis in 3487 Center Ambulance calls, cards (Form 110u)
for the period 2008 - 2010 (the month of June), at the age of 18 to 65 years who have received
pre-hospital medical care. As a result of which, it was noted that doctors increasingly began to
use modern injectable antihypertensive drugs - ACE inhibitors, -blockers, -blockers, at least
- -blockers and centrally acting blockers of Ca channels.

Adiponectinemia inhypertensive patients depending oncardiovascular complications risk
Abstract
Purpose - a comparative investigation of the concentration of adiponectin in hypertensive
patients with varying degrees of risk of cardiovascular complications. Materials and Methods.
68 patients with I, II grade arterial hypertension aged 35 to 56, divided into 3 groups depending
on the risk of development of cardiovascular complications, underwent examination,
including echocardiography, ultrasound dopplerography of brachiocephalic vessels , daily
blood pressure monitoring, lipid spectrum, glucose level and adiponectin concentrations in
blood serum. Results. In hypertensive patients with low risk of cardiovascular complications
the concentration of adiponectin decreased by 32% (p <0,05) compared with those in the
control group. Statistically significant negative correlation between adiponectin and low
density lipoproteins levels (r = - 0,234), triglycerides (r = - 0,518), body mass index (r = - 0,554)
indicates the relationship of inflammatory processes and the impairment of lipid metabolism
in hypertensive patients with average risk of cardiovascular complications. In the subgroups
of high risk patients the level of adiponectin was significantly low.

The influenceof implantationcava filter on the hospitaltreatment resultsof patients withpulmonary embolismand a high-intermediate risk
Abstract
The article devoted to the study of pulmonary embolism and a high-intermediate risk in
117 patients ranging in age from 48,5 to 65,3 treated at the Samara regional clinical cardiology
dispensary. An assessment of the influence of implantation cava filter in the inferior vena
cava in addition to standard therapy with heparin or thrombolytic on the hospital treatment
result. It is shown that the implantation of cava filter does not influence on the degree of
pressure reduction in pulmonary artery applied by standard methods of treatment and does
not contribute to the development of a higher incidence of residual pulmonary hypertension
at the time of discharge from hospital, while reducing hospital mortality rate regardless of the
main and common method of treatment (thrombolytic therapy or standard heparin one).

The gender-geneticassessment of frequencyoccurrence of diastolicdysfunction and types of hypertonicremodeling in patientswith controlledand uncontrolledarterial hypertension
Abstract
In this investigation there was revealed the interconnection of the hypertonic remodeling
type and the difficulty of breaking diastolic function from genotypes characterizing the flow of
arterial hypertension. 76 persons have been investigated: 42 persons with controlled arterial
hypertension, 34 persons with uncontrolled arterial hypertension. All patients were compared
by age and sex. The investigation of echocardiography by using the apparatus Medison Sonoace
8000 was conducted. The molecular genetic investigation with determining genotypes of
polymorphic genes ACE, AGT2R1, eNos was also conducted for all patients. So, the connection
of diastolic dysfunction of the heart at women with uncontrolled arterial hypertension is
associated with concentric hypertrophy of the left ventricle. The difficulty of the diastolic
function and the prognosticly unfavourable type of remodeling heart are associated with
genotypes: ACE - II, AGT2R1 - AG, eNos - ab.

The lengthof service influence on course severityof essential arterial hypertensionin locomotive team employees
Abstract
The article provides assessment of the impact of work experience on the progress and course
of essential arterial hypertension in locomotive team employees. The survey group consists of
160 patients Samara railway station hospital based on the Russian railways including 120
employees of locomotive teams with essential arterial hypertension stage I-II and 40 railway
employees from other occupation, which is not related to train safety (engineers and office
employees) without cardiovascular disease. It is shown that the severity of essential arterial
hypertension depends on the duration of the working experience.

Abstract
Step by step complex cardiological rehabilitation is the effective method of the restoring
treatment of patients with acute myocardial infarction. It contributes to an increase of the
physical capacity for work, improves the clinical course of disease, slows down the progression of
atherosclerosis, stabilizes psychological status and it restores the patients ability to work.
The analysis of the results of the rehabilitation of patients with acute myocardial infarction
in 14 years (from 1997 until 2010) under the conditions of the department of the restoring
treatment of the polyclinic ¹15 of Samara is presented.

The diagnosticvalue of the leukocytic indices ofcellular reactivity inan assessment ofmyocardial infarction healing
Abstract
Predictive value of the integral leukocytic indices on the basis of leukocyte counts of
peripheral blood in 400 patients and 84 patients who died from myocardial infarction (MI)
was determined. Ñontrol group consisted of 1064 people. A significant (p <0,01) increase in
leukocyte shift index of blood (LSI), the ratio of neutrophils and lymphocytes (NLymRI) and
leukocyte intoxication index (LII) in patients with myocardial infarction compared to control,
as well as in groups of patients with transmural end non-transmural MI were established. There
were no differences in leukocyte integral indicators, depending on age and sex of patients.
Recommended reference values ISLK, NLymRI and LII for use in clinical practice for the
diagnosis of complicated MI.

The influence ofgenetic polymorphismsof CYP2C9 andVKORC1 on warfarin dosingin patientswith permanent formof atrial fibrillation
Abstract
The article is devoted to assessing the influence of genetic polymorphisms of CYP2C9 and
VKORC1 on warfarin dosing in patients with permanent form of atrial fibrillation. Studied
were 100 patients with coronary heart disease and permanent form of atrial fibrillation,
mean age 60,5±5,8 years. In patients with permanent form of atrial fibrillation genotypes
CYP2C9*1/*3, TT and GG genotypes of VKORC1 are associated with low warfarin dose. Genotype
CYP2C9*1/*3 increases the risk of bleeding complications during warfarin therapy.

Quality of life ofpatients with operatedtraumatic intracranialhematomas
Abstract
The ñraniocereberal trauma is an actual problem of modern neurology and neurosurgery
that is connected with high frequency and weight of consequences of this pathology. One of
the types of heavy ñraniocereberal trauma is the traumatic intracranial hematoma. A research
objective was studying of quality of life of patients in the remote period of surgical treatment
traumatic covering hematomas. 120 patients ranging in age from 18 to 69 to terms from
6 months till 3 years after a trauma were examined. For definition of quality of life scale
SF-36 was used. The conducted research has shown that that in the majority of supervision
satisfactory indicators of quality of life are received. Dependence between quality of life of
patients in the separated period and such factors, as localization, volume of hematomas and
degree hypertensive-dislocation syndrome are revealed. The best indicators are recorded in
group of patients with hematomas of small volume and with chronic subdural hematomas,
and as operated in terms during 24 hours after a trauma.

failure, functional class I-II (NYHA), combined with duodenal ulcer in remission.

The epidemiologicalfeatures of multiplesclerosis in theUlyanovsk region
Abstract
Analytical study of multiple sclerosis (MS) has been carried out with respect to international
methodological criteria in the Ulyanovsk region; 127 pairs of MS patients were examined (81%
female). The study shows that general MS risk factors for the taken population are: prolonged
work with computer equipment, living near to construction materials production facilities
under the age of 15, chronic herpes infection for all age categories, chronic bacterial infections
(tonsillitis, sinusitis, abscesses) preceding to MS development, head injuries without loss of
conscience under the age of 15 and prolonged contact with home animals for patients over
the age of 15.

Features of cellularcomponent of a system inflammationin patients withmoderate and mildform of ñhronicobstructive pulmonarydisease (COPD) in acute stage
Abstract
In this article we cite data which demonstrate cellular component of a system inflammation
in patients with moderate and mild stage of ñhronic obstructive pulmonary disease (COPD)
in dynamics of hospital therapy during the exacerbation of the given disease. Control group
consist from 20 conditionally healthy faces. The received results demonstrate that COPD exacerbations
in patients with moderate and mild stage is characterized by strengthening phagocytic
activity against decrease in number of such cells. Also at such patients activation Ò and B
cells, against the raised level of the T-NK-cells that depict presence of chronic inflammation
at patients already with mild stage is observed. At patients of moderate stage the exhaustion
of NK-cells is observed. It speaks about easing of antiviral and antineoplastic immunity.

Hemostasis impairmentin patients with erythremia and multiplemyeloma in a combination with coronaryheart disease
Abstract
On the basis of the Samara State Medical University Clinic 53 medical cases hematological
malignancies (erythremia and multiple myeloma) were analyzed. Coronary heart disease
(stable angina) occurred in accordingly 65% and 59% of cases.
In the study of hemostasis (PTI, INR, ADP-platelet aggregation, collagen-platelet
aggregation, platelet aggregation with MAP) a tendency to hypercoagulability in most patients
was marked, mainly in patients with erythremia, the most specific test is the ADF-platelet
aggregation. In view of revealed changes in these patients coagulation study and prevention
of complications associated with the development of thrombosis should be enhanced.

Superficial facialelectromyography as a techniqueassessment of tuberculin diagnosis
Abstract
For a choice of a technique of statement of optimal physiologically comfortable sample
psychophysiological reactions of an organism of a child according to a superficial facial
electromyography on Mantoux test reactions and prik-test has been studied. Performance
of intradermal Mantoux test reactions with 2ÒÅ in children has taped the stressful reaction
accompanied by negative emotional reaction. The offered technique prik-test has smaller
irritating effect. It is low stress factor for children in comparison with a Mantoux reaction in
case activation of parameters of a facial electromyogram in the first case occurs less on 67%
on power and on 66% on amplitude. For carrying out of a mass tuberculin diagnosis the priktest
is more optimal method.

The changesin the peripheral blood ofHIV-infected patients with severecommunity-acquired pneumonia
Abstract
The article investigates the changes in the peripheral blood of HIV-infected patients with
severe community-acquired pneumonia. It was found that pneumonia in the late stages of HIV
infection is characterized by anemia, leukopenia, a more pronounced shift to stab, decrease in
the number of lymphocytes, monocytes, and increase of segmented neutrophils, higher levels
of ESR. The study identified the relationship between the intensity of changes in the blood
and the severity of pneumonia.

Effectiveness of ozonotherapyin the complex treatment ofchronic cystitisin women
Abstract
We surveyed a group of patients with chronic cystitis. The aim was to study effectiveness
of ozonotherapy in the complex treatment of such patients in the microcirculation of the
bladder wall, and change the composition of the microflora of the lower urinary tract. The
study involved 65 patients aged from 20 to 86. Patients were divided into three groups: the
first group of patients (n=20) received complex conservative therapy, the patient of the second
group (n=30) received, along with basic therapy intravesical instillation of ozonized solution,
the third group of patients (n=15) was carried parenteral injection of ozonized saline. The use
of ozonotherapy in the treatment of chronic cystitis in women has a pronounced bactericidal
effect, causes an improvement of microcirculation in the bladder wall, leading to rapid
disappearance of dysuric disorders.

and microbiological aspects of community-acquired pneumonia in adults. In the work the
epidemiological data and pathogenetic mechanisms of development of community-acquired
pneumonia are resulted, diagnostic methods of disease are presented. The key moments of
immunological reactance of an organism are reflected at the given disease. The data available
in the literature on structure of pathogens of community-acquired pneumonia and modern
indicators of antibiotic resistance in pathogenic strains are resulted.

Abstract
The article analyzes the course of pregnancy and childbirth in women - workers of mental work. The influence of professional educators on the reproductive health of women during gestation, birth outcomes and neonatal state are shown. Complicated course of gestation: 2 times more likely to occur gestosis, intrauterine growth retardation are revealed. Deliveries are marked by fast term and the increase of surgical interventions in four times.

Realization predictionof neonatal infectionsin children of early age
Abstract
The article presents literature data on the problem of babies' antenatal infection realization predictions mother's risk factors are discussed (mother's health peculiarities, gynecological anamnesis, present gestation and maternity development, placenta condition), as well as ways of infection carry-over to a fetus. Characteristics of fetus and infant condition favoring antenatal infection realization are also described. The necessity of complex approach to the antenatal infection realization risk estimation, which will allow decreasing infant mortality, is proved.

Abstract
For specification of diagnosis JIA and definition of the importance of pathogenetic changes research of parameters humor immunity, the most informative is shown at the given pathology, namely: a spectrum cytokines: TNF α, IL6, IL1β, IL4, a level spontaneous both stimulation lymphocytes and monocytes - TNF α. Value cytokines is determining in an inductive phase of the immune answer when they cause development of base reactions makrophages and antigen-specificaition cells: interaction of these cells with lymphocytes, activation and proliferation the last, in the certain degree - formation of cells - effector. At late stages of the immune answer, during realization - effector immunity functions the role cytokines becomes less significant. Level both stimulations lymphocytes and monocytes TNF α reflects activity of pathological process and can be used in forecasting outcomes juvenile idiopathic arthritis.

Features ofphysical development of the prematureinfants during the first year of life
Abstract
Physical development indicators of 109 premature infants born between 29 and 37 weeks gestation have been studied. It has been shown that during the first year of life, body weight of premature infants is less than that of full term newborns of the same age. During the second half-year of life, according to the indicators of body length, head and chest circumference "moderately premature infants" (GA 32-36 6/7 weeks) catch up full term babies. Very premature infants (GA 29-31 6/7 weeks) demonstrate the lowest indicators of physical development combined with high growth paces of basic anthropometric parameters. We have determined succession of leveling indicators of physical development of premature infants: chest circumference, head circumference, length, body weight.

Abstract
In order to optimize the treatment of children with torpid course of bronchial asthma the study was conducted, which confirmed the presence of certain features of bronchial asthma in children with colonization and inflammatory reaction of the mucous membranes of the upper respiratory tract. The data obtained substantiate the need for prompt diagnosis of microbial contamination and remediation of the upper respiratory tract infection (TTP) that will optimize treatment, reduce the duration of asthma exacerbations in children with bacterial colonization of the mucous TTP.

Abstract
It is clear that the idea of vaginal infection, as a purely isolated processes that do not involve reproductive disorders and complications of pregnancy, is gone. The suppression of colonization resistance of vaginal microflora contributes to the rising infection endometrium and fallopian tubes and development of infectious and inflammatory diseases of the pelvic organs, which can not affect the reproductive health of women planning pregnancy.
The problem of violations of the biocenosis of the vagina during pregnancy in obstetric practice is relevant, as changer in vaginal microflora lead to complications during pregnancy, childbirth and the postpartum period, as well as the development of infections - inflammatory processes in the mother and fetus. We set out to examine violations of vaginal biocenosis in pregnant women and to analyze the outcome of pregnancy for both mother and fetus. We undertook a prospective study of 131 women. All women were examined according to accepted standards and in our developed algorithm in taking them on account of pregnancy.

Predictors ofcomplicationdevelopments after liver resections
Abstract
Results of management of liver mosses in 159 patients treatment of 159 patients are presented. Routine blood analyses, duplex Doppler ultrasonography, reohepatography, intraoperative conditions were compared for predicting postoperative liver dysfunction and bile complications. We concluded that measurenant of the portal blood flow relocity, the volume of the liver resection and the intraoperative bluding are correlated with the development of the postoperative complications.

Possibilitieslow-intensive laser radiationapplication in preoperativepreparation of venous trophiculcers of lowerextremities
Abstract
The basis for the work is based on the results of treatment of 149 patients with chronic venous insufficiency of the lower extremities 6 class (the system of CEAP, 1994). In the comparative aspect of the efficiency of the method of preoperative preparation of venous trophic ulcers of the traditional way and with the use of low-intensity laser radiation with a simultaneous spray liquid medicines depending on the phase of the wound process. The obtained results show that the use of low-intensity laser radiation has significant advantages in comparison with traditional methods of preparation of venous trophic ulcers to surgical intervention, as it stimulates the reduction of terms of preoperative preparation of various in size and degree of changes of the surrounding tissue ulcers defects.

Abstract
Chronic ischemia of the lower extremities caused by obliterating atherosclerosis is characterised by suppression of T-cell immunity, disbalance of indicators of humoral protection, components of complements´s system, activation of peroxide lipid oxidation, suppression of antioxidant protection. The use of autohemoozonotherapy in structure of complex conservative therapy, unlike traditional therapy, allows normalizing the majority of the investigated indicators of the immune and antioxidant status.

Assessment of thethe application of video-assistedmini-thoracotomy in patients withchest injuries complicated withpneumothorax and hemothorax inthe city clinical hospital
Abstract
After the application of video-assisted mini-thoracotomy in 85 patients with chest injuries, aged from 15 to 92 the analysis has been done. The patients were divided into 3 groups, depending on the complication of the injury. 43 patients with posttraumatic pneumothorax, amounting to 50.6%, entered Group 1. 12 patients with posttraumatic hemothorax with suppressed or ongoing haemorrhage, amounting to 14,1%, entered Group 2. Group 3 enlisted 30 patients, amounting to 35,3%, being operated a week after they were taken into hospital. Mini-thoracotomy was applied to all patients with lateral incision of 6-8 cm long, 41% of patients had lengthened incisions up to 12cm because of technical difficulties. Thoracoscopy was carried out with standard equipment, rigid thorascope being used on 54 patients, amounting to 63,5%. 5 patients (5,9%) with severe concomitant injuries died. The effectiveness of video-assisted mini-thoracotomy for patients with chest injuries has been underlined.

Abstract
They have generalized experience of treatment of 58 patients with traumas and strictures of hepaticocholedochus after operation after formation of biliodygastive anastomoses, including 6 patients with removable transpechenochnom anastomosis by Getts. All the patients after operations had exacerbation of cholangitis, but only 5,2% had lingering nature. When the treatment of acute and chronic cholangitis included infra-red laser exposure (0,63 mkm wave-length and power of 20 milliwatt) set up in drainage, it helped to arrest inflammation in patients of that category.

Abstract
The effectiveness of the Diaskintest for detecting tuberculosis in 61 children and young people in the Children's Hospital of Samara city was studied. The most frequent forms of tuberculosis were intrathoracic lymph node tuberculosis (39, 3%) and infiltrative tuberculosis (27,9%).
The Diaskintest® has shown its high diagnostic importance when dealing with local forms of active tuberculosis (88,5%). The negative test result by Diaskintest® does not exclude presence of tuberculosis. The Mantoux test remains important diagnostic test for detecting tuberculosis.

From obtained diagnostic laboratory value, sensitivity and specific factor the following data were special IgE to a mix of pollen of trees of black-earth zone (AUROC ≥0,7), IgE to a mix of pollen of meadow herbs, IgE to a tick Dermatophag, IgE к Asperg, IgE total (AUROC >0,5), and eosinophiles and monocytes of peripheral blood (AUROC >0,5).

Metabolites levelin blood and enzymes activity inthe liver at acute alcoholicintoxication in ratswith experimental diabetes
Abstract
On the model of acute alcoholic intoxication at a singl dose 4 g/kg in rats is shown that in 6 hours in blood the level of free fat acids, urea is increased. In 24 hours concentration of glucose, lactate and urea is increased, the level of free fat acids decreases. Activity of glutamatdehydrogenasa and tirosinaminotransferasa in liver at acute alcoholic intoxication increases. In the rats with experimental diabetes (15 day) in 6 hours after an injection of ethanol in blood the level of glucose, urea, triglycerols is increased, the concentration of free amino acids, lactate is lowered. In this condition in liver of experimental animals activity of glutamatdehydrogenasa decreases and tirosinaminotransferasa increases. In 24 hours in blood concentration of triglycerols is lowered very much, the level of urea remains increased.

The expression of genes NO-synthesisat a trauma of locomotorsystem
Abstract
In the experiment with unthoroughbred male rats the expression of genes NO-synthesis and morphometric indicators of vessels at a trauma of locomotor system at a damage zone were studied. Duration of the experiment was 1 and 14 days. It is established that the expression of genes NOS1 and NOS3 was extremely low in all experimental groups. While authentic reduction of level of the expression of gene NOS2 in 2,4 times to control group was noted after 14 days after the trauma.

Prognostic valueof gene of HLA-DRB1in chronic myeloid leukemia
Abstract
In this research work we studied the HLA- DRB1 gene in patients with chronic myeloid leukemia (CML), who have been treated with Gleevec for 24 months. The study found that the markers of poor prognosis of CML, which predict failure to achieve a complete cytogenetic response after 24 months of Gleevec therapy are the specificities DRB1*14 (06); DRB1*11 (05); DRB1*12 (05). The markers of favorable prognosis of CML, which characterize the achievement of complete cytogenetic remission after 24 months of Gleevec therapy are DRB1*15 (02); DRB1*16 (02).
