Comparative analysis of the clinical efficiency of conservative myomectomy and uterine artery embolization as methods of treatment of uterine leiomyoma
Abstract
The research included 92 patients of reproductive age with uterine leiomyoma, who underwent conservative myomectomy and uterine artery embolization. Among the clinical manifestations of uterine leiomyoma the most frequent were pain, menometrorrhagia and infertility. It was found that uterine artery embolization has an advantage over the conservative myomectomy due to significantly shorter duration of operation and hospital stay, absence of need for anesthesia care, and absence of intraoperative complications. Authentic positive clinical results were obtained 5 months after both interventions. However, after conservative myomectomy more rapid regression of such clinical manifestations as pain syndrome and menorrhagia was observed.

Clinical and laboratory characteristics of women's health prior to the detection of pathological processes of the exocervix
Abstract
The author had carried out a retrospective cohort analysis of the medical histories of women diagnosed with abnormal cervix uteri, living in an environmentally disadvantaged area of the city and in a residential area with more favorable environmental conditions. The direct moderate correlation between the increase of emissions of harmful substances into the air and the growth of the number of cervical diseases has been revealed. It has been shown that the development of cervical dysplasia in patients occurs against the background of gynecological and somatic diseases. The high incidence of the pathology of the reproductive system, pregnancy, labor as well as associated somatic diseases in patients with diseases of the cervix uteri could largely be attributed to the peculiarities of the pathogenic influence of the environment on the whole organism of the patient.

Clinical realization of violations of trophic and gas exchange functions of placenta within placental issufficiency
Abstract
The purpose of the study was to assess the presence of chronic hypoxia with growth retardation of varying severity on the basis of the analysis of clinical and laboratory data and instrumental examination of pregnant women with high risk of decompensation of placental insufficiency. 120 pregnant women with high risk of severe forms of placental insufficiency were examined. Examination of pregnant women included ultrasound, Doppler ultrasound, cardiotocography, determination of unconjugated estriol and progesterone. In newborn's cord blood pH, pO2 and pCO2 were determined, and the parameters of physical development, taking into account standards of percentile tables, were evaluated. Analysis of gas exchange disorders and trophic functions of the placenta showed that chronic hypoxia in 100% of cases was detected with growth retardation II, III degree, and only 86.4% with growth retardation I degree.

The evidence of pathogenetic link between embrio(feto)-placental insufficiency with early and late preeclampsia
Abstract
The aim of the study was the identification of common pathogenic mechanisms for the formation of PN and PE to justify the selection of a single method of prevention of these complications of gestation. Based on a dynamic survey of 70 pregnant women at high risk of the development of severe forms of MON, we analyzed the common reactive potential of the organism, the markers of endothelial dysfunction, apoptosis, inflammatory response, decidualization, and cell transformation: common predictors changed 3-6 weeks before the implementation of PN and PE, which justifies the use of unified preventive approaches.

The results of magnetic-laser therapy for acute purulent rhinosinusitis
Abstract
In the article, the effect of magnetic-laser therapy on physical status of patients with acute purulent rhinosinusitis and nasal functions is revealed. We examined 100 patients with acute purulent rhinosinusitis. The first group (n=50) of patients with acute purulent rhinosinusitis got only basic therapy; the second group (n=50) of patients with acute purulent rhinosinusitis got basic therapy and magnetic-laser therapy. All patients were questioned with SNOT-20, got special examination, had their nasal function checked. The investigation revealed positive treatment effect of magnetic-laser therapy for acute purulent rhinosinusitis during the first five days. So, we can recommend five-day course of magnetic-laser therapy for acute purulent rhinosinusitis.

Computed tomography with the use of system of image digital analysis in patients of COPD risk group
Abstract
We studied morphological changes in the lung tissue by visual analysis of CT scans and using the developed program. The similarity of changes in groups of COPD and chronic nonobstructive bronchitis was stated; signs of mild bronchial obstruction in patients from the group with chronic nonobstructive bronchitis were revealed.

Modern approaches to the diagnosis and treatment of renovascular hypertension (literature review)
Abstract
The article is devoted to the review of the actual information about diagnosis and treatment of renovascular hypertension. The clinical significance of renovascular hypertension is determined by high prevalence of this disease and its severe negative impact on prognosis. Despite the existence of a large number of studies in this field, at present there is no universally accepted treatment strategy for patients with renovascular hypertension, which continues to be an actual problem.

Selective laser trabeculoplasty in the treatment of patients with primary open-angle glaucoma
Abstract
The article analyzes the results of selective laser trabeculoplasty in the treatment of patients with early, advanced and extensive stages of primary open-angle glaucoma. The best results are obtained in patients with primary and advanced stages of glaucoma. In patients with extensive stage of glaucoma, intraocular pressure reduction after laser treatment does not permit to achieve the target pressure, and more intensive antihypertensive therapy or surgical treatment are required.

Laser treatment of diabetic macular edema
Abstract
The article considers the threshold and sub-threshold methods of mi-cropulse laser treatment of diabetic macular edema. Attention is paid to the mechanisms of producing changes in the retina and therapeutic effect in each of the methods. The study revealed their strengths and weaknesses, as well as methods of delivery of laser radiation on the retina, including navigation methods.

Pracice of surgical treatment of uveal cataract in children
Abstract
The course and the results of surgery treatment of uveal cataract in children are studied in the article. The results of surgery treatment of 12 patients (15 eyes) were analyzed. Facoemulsification with implantation of intraocular lens was made for all the patients. The improvement of visual functions and lessening of the recurrence rate were achieved in postoperative period.

Clinical epidemiological analysis of dynamics of eye deseases incidence and hospital ophthalmological aid to the population of Samara region during 2009-2013
Abstract
The clinical epidemiological analysis of dynamics of eye diseases inci-dence during 2009-2013 allowed the authors to identify the growth of oph-thalmological pathology of the population of Samara region, primarily due to cataract, glaucoma, and retinal diseases. At the same time, the number of ophthalmologists in polyclinic decreased and, subsequently, the number of ophthalmologist’s consultations also decreased. However, the indicators of the hospital ophthalmological care are improving from year to year. This analysis helped to develop a regional target program of improvement of ophthalmological aid to the population of Samara region.

Outflow and ocular rigidity in human eyes: experimental research
Abstract
The experiment with 34 human enucleated eyeballs investigated the effect of the rigidity of the eye on the outflow of anterior chamber with the decrease of intraocular pressure from 30 mm Hg up to 15 mm Hg. The average stiffness of the eyeballs was R = 0.026 ± 0.002 mm Hg / mm3. The time of decrease of the intraocular pressure ranged from 3.3 to 18.4 minutes, at an average of 9.0 ± 1.3 minutes. Coefficient of ease of the outflow in study group eyes averaged 0,090 ± 0,006 mm3 / mm Hg / min. In the group with the axis of the eyeballs less than 24.0 mm R = 0.03 ± 0,003 mm Hg/mm3, the outflow was C = 0.10 ± 0.08 mm3 / mm Hg / min. In the group with the axis more than 24.0 mm R = 0.019 ± 0.002 mm Hg / mm3, the outflow was C = 0.067 ± 0.07 mm3 / mm Hg /min. Influence of age-related changes on the rigidity of the outflow was not identified in this study. It has been shown that with the increase in the rigidity of the eye the speed of fluid ejection from the anterior chamber increases, and vice versa - with decreasing rigidity the outflow rate of moisture from the anterior chamber decreases.

Surgical treatment options for symptomatic vitreomacular adhesion and its complications
Abstract
In the article, features and results of surgical treatment of vitreoretinal surgery in patients with symptomatic vitreomacular adhesion and its complications are studied. We analyzed the results of surgical treatment of 124 patients with symptomatic vitreomacular adhesion complicated by full macular hole, epimacular membranes, lamellar rupture and foveoschisis at the age of 51.4±2,4 years. The use of modern minimally invasive vitreoretinal surgery, including chromovitrectomy, allows us to obtain high anatomical and functional results in the treatment of patients with symptomatic vitreomacular adhesion and its complications in the early and remote period of observation. Intraoperative and postoperative complications, anatomical and functional results were analyzed. Modern microinvasive vitreoretinal technologies, including chromovitrectomy, are much easier and make the surgery of symptomatic vitreomacular adhesion and its complications safe, predictable and effective.

The article examines the problems of treatment of subretinal neovascularization in the presence of open-angle glaucoma of I-II stage. The remote results of treatment of patients with subretinal neovascularization associated with age-related macular degeneration and open-angle glaucoma of I-II stage are analyzed. The assessment of influence of stabilization of intraocular pressure upon the dynamics of visual functions is carried out.

The role of immunological parameters in non-invasive diagnosis of hepatic fibrosis in patients with chronic hepatitis C
Abstract
The work is devoted to the improvement of non-invasive diagnosis of liver fibrosis in patients with chronic hepatitis С by introducing immunological tests into the arsenal of existing methods. It was found that immunological tests showed great diagnostic significance at the beginning of fibrotic changes. For the diagnosis of stage F3 of liver fibrosis (METAVIR scale) and liver cirrhosis a new Immunofibrotest, which exceeded the diagnostic accuracy of traditional non-invasive methods, was proposed.

Effectiveness of radiofrequency ablation for treatment of lung cancer
Abstract
The aim of this study was to analyze the results of our radiofrequency ablation (RFA) procedures of primary non-small cell lung cancers (NSCLC) to appreciate the effectiveness of this method. Twenty-eight lesions in 28 patients who underwent RFA from April 2008 to December 2009 were analyzed (20 - adenocarcinoma, 5 - squamous cell carcinoma, 2 - neuroendocrine carcinoma, 1 - clear cell carcinoma). On the basis of the clinical-radiological alterations that were revealed at the first computed tomography (CT) scan and at the subsequent computed tomography, we evaluated the success of total ablation, local control of tumor and one-, three- and five-year survival rate. The complications after RFA were also evaluated. At the first CT scan complete ablation (CA) was obtained in 100%. Nine patients (32.1%) had local recurrence during the follow-up (48 months on the average). Overall survival at 1, 3 and 5 years was 89.2%, 67.7% and 39.5 %, respectively. There were no major complications. Pneumothorax occurred in 4 (14.3%) out of 28 treatment sessions: 1 was asymptomatic, and 3 required thoracostomy. Pleuritis occurred in one (3.6%) session and one more patient (3.6%) had lung abscess in the RFA area. Conclusion: Radiofrequency ablation of NSCLC was effective, well tolerated, with satisfying overall survival. However, to determine the role of RFA in the treatment of patients with non-small cell lung cancer, we need to compare RFA with surgery and radiation therapy.

Assesment of local tumor control after radiofrequency ablation in patients with lung cancer
Abstract
Radiofrequency ablation (RFA) in primary and metastatic lung cancer is a relatively new medical procedure, and currently there are no clear criteria for assessing the changes occurring as a result of RFA. It is necessary to define a set of features characterizing the zone of postablation lesion and subsequent fibrosis formation to estimate the success of this minimally invasive surgery and local control of dynamic changes in the lung. From April 2008 to December 2012 in Samara regional clinical oncology center radiofrequency thermoablation of lung tumors was performed on 306 lung nodules in 221 patients with primary and metastatic lung tumors. All patients underwent computed tomography (CT) before and after RFA. The authors have developed criteria for evaluating postablation changes based on the analysis of the data.

Sphincter modelling methods in formation of end colostomy
Abstract
This article is devoted to the problem of choosing the optimum way of end colostomy creation in colorectal cancer patients. Uncontrollable colostomy function, possibility of paraostomy complications are significant negative factors decreasing the quality of life. The problem is not only in uncontrollable functioning of the ostomy, but in inability to feel coming defecation. Results of various techniques of ostomy creation were studied and compared with proposed technique of a neosphincter creation after obstructive resections of colon and rectum. Frequency, duration of depletion, need of application of laxative preparations and cleaning enemas were studied. It was shown, that the development of postoperative paraostomy complications depends on the way of its creation. The offered way of colostomy formation using sphincter modelling technique allows us to decrease postoperative complications rate and considerably improve the quality of patients’ life.

Practice of intraoperative ultrasonography during thoracoscopic surgery
Abstract
When using video-assisted thoracoscopy (VАТS) concerning the presence of focal lesions in the lungs, intraoperative ultrasound can be used for finding abnormal focus. The article presents the results of the practical use of this imaging technique when performing surgery for peripheral solitary pulmonary structures in 47 patients. The analysis of the effectiveness of intraoperative ultrasonography was carried out in comparison with the classical methods of searching for intraparenchymal abnormal foci with VАТS.

Examination of mutational status of cancer and the survival rate after combined therapy of patients with metastatic colorectal cancer
Abstract
In order to develop the right therapeutic approach for patients with metastatic colorectal cancer under treatment, it is necessary not only to define the prevalence of neoplastic process, but also to estimate biological activity of the tumor. Mutational status of KRAS gene has the key value in assessment of biological activity of the tumor. Thus, it can have an impact on the effectiveness of treatment and can be regarded as prognostic factor of analysis of long-term results of combined therapy in patients with metastatic colorectal cancer.

Intraperitoneal plastics of postoperative and umbilical hernias
Abstract
The article describes a method of performing intraperitoneal plastics of umbilical and postoperative hernias, its technical features, as well as the evaluation of the performance of the proposed method, comparing it to previously proposed methods of classical hernia repair. Operation without the mobilization and removal of the hernia sac eliminates unnecessary trauma to the tissues, and thus reduces the number of postoperative complications. We open only the upper pole of the hernia sac, which serves as the entrance into the abdominal cavity. An intraperitoneal plastics was performed inside the hernia sac. The application of intraperitoneal plastics enabled operating with the smaller cuts. When choosing the length of the cut, the size of the hernia sac did not matter, but the size of the hernial orifice was important. Operation via the hernia sac cavity created quite a comfortable environment for the implementation of intraperitoneal plastics with a section corresponding to the size of the hernial orifice. During the surgery we avoided any contact endoprosthesis with subcutaneous fat. After fixation of the prosthesis, the layers of preserved hernia sac were stitched over it. In this case, the peritoneal surface of the hernia sac was de-epithelialized, and firm adherence of de- epithelialized peritoneum to the surface of the implant was created. This was important for the prevention of wound complications. The proposed method of muscular-aponeurotic plastics with endoprosthesis replacement creates the conditions of high mechanical stability combined with the properties of elastic decompression.

Improved technique of taking impression
Abstract
This study estimates the accuracy of taking an impression of an implant or a tooth with the use of the classical and the improved techniques. During this type of impression taking, preparation zones on natural teeth are often displayed inaccurately (there are stretchers, incorrect image of the shoulder and the stump in the impression). In this regard, we have posed the following research objective: to evaluate the accuracy of the image using the improved method and the classical method.

Modern aspects of prosthetics of patients with low and destructed clinical teeth crowns (review)
Abstract
The problem of preservation of teeth with lost or damaged part of the crown in case of low clinical crown is relevant in terms of restoration of the broken morphofunctional unity of dentition. The aim of the study was the analysis of modern methods of treatment of patients with low and destroyed clinical crowns. The authors studied Russian and foreign literature sources devoted to this problem. The analysis revealed the lack of consensus about the tactics of treatment of patients with low and destroyed clinical teeth crowns, which actualizes further improvement and search for new methods of prosthetics of patients with this pathology.

Influence of stomatological health upon the quality of life of HIV-infected patients
Abstract
The parameters of quality of life of 107 stomatological patients with HIV infection were investigated. Three groups of dental health were allocated. On the basis of correlation dependence it was proved that stomatological diseases sharply reduce the quality of life and health. A patent for the program «Indexes of dental health" was received.

Evaluation of bioplastic material Hyamatrix in vitro on the culture of mesenchymal stromal cells of human bone marrow
Abstract
The experiment hasn’t revealed any adverse or toxic effects of the material on a population of cells. On the contrary, the proliferative activity of the study data showed its stimulation. These data lead to the conclusion that the analyzed material Hyamatrix is biologically active, increases the proliferative capacity of cells, is biocompatible, provides cell adhesion to the surface, and shows no negative influence on the morphological characteristics of the cells.

Health status of city working population
Abstract
The study shows a reduction in the incidence of temporary disability of the working population of Kuibyshev district of Samara in the period from 2006 to 2013 counted in the number of cases and in the number of days, in general and in most classes of diseases, meanwhile the average duration of cases of temporary disability has reduce, maintaining its structure. Self-evaluation of the health status of the urban working population is slightly higher than a satisfactory value. Subjective assessment of morbidity with temporary disability by working respondents in cases and days is fourfold higher than the official statistics. At the same time, the structure of morbidity with temporary disability, obtained by sociological study and according to official figures, have significant similarities.

Analysis of performance indicators of healthcare system in the Volga Federal district
Abstract
This article presents an analysis of the major trends in the healthcare system of the Volga Federal District. It was found that currently there are about 100 000 doctors of different specialities in the district; the average amount of doctors per 10 000 people is 39.1. In the regions with the greatest area of the territory, there is imbalance in medical and nursing staff suffi-ciency (between the regional and district centers, municipalities, urban and rural areas). The average birth rate in the regions of the district is 13.39 per 100 thousand of population; the average mortality rate reaches 13.82 per 100 thousand of population. In the analysis of cancer morbidity and mortality indicators, a significant scatter of values in different regions of the district and a serious deviation from the mean values of the district were revealed.
